Hydrocortisone 1%, Miconazole nitrate 2% chlorhexidine 5% in Ava creme?

Chlorhexidine 5%, that's the diluted 20% anyway?

Answer

I do not suspect a problem with miconazole nitrate but with chlorhexidine. Indeed, 5% of the 20% solution is prescribed. But Chlorhexidinedigluconate has O/W emulgatorial Properties!

I would suggest to convert the Diglucnaat to the water insoluble dihydrochloride, which is also used in specialties such as antiseptic.   A small amount of NaCl (26 mg per   20mg of chlorhexidine) may already be sufficient. You must prepare AVA without sorbic acid. Due to the presence of Chlorhexidine no other preservative is required.
