We received the following prescription: Tretinoine 7.5 mg Cremor non ionicus aquosus ad 30g

What's the most accurate and stable way to prepare this CR è me? Can a trituration of tretinoine be made in lactose or mannitol? And will it remain stable for a long time? Or should a solution be made in alcohol (it solves very badly, and the amount of alcohol will be too much to process in this CR è me)? Should an anti-oxidance be added in any case?

Answer

Taking into account my latest publication in the Belgi Pharmaceutical journal ë on the processing of hazardous substances, I should advise you to prepare the hydrophilic trituration described there. Tretinoine is a dangerous substance and we need to avoid contact with dust particles. As described in the article, the use of a solution in e.g. alcohol is not advisable. With the 2% hydrophilic concentrate you can prepare a non ionogenic CR è me without any problems. You already have cetostearyl alcohol to which the remaining components of the non-ionogenic CR è me have to be added. The requested anti Oxidance is provided.
